REQUIREMENTS FOR KINDERGARTEN
- Physical Exam & Health Questionnaire Completed
- State of NJ Mandated Immunizations
- 4 doses DTP (Diphtheria, Tetanus, Pertussis) with 1 dose given after 4th birthday or 5 doses
- 3 doses Polio (OPV) with 1 dose given after the 4th birthday or any 4 doses
- 2 doses of MMR (Measles, Mumps, Rubella)
- 1 dose of Varivax/Varicella (Chicken Pox) dose must be administered on or after 1st birthday
- 3 doses of Hepatitis B (series of 3 injections)
NJ Immunization Chart - Information from the NJ Department of Health on required Immunizations for school-aged children and in which grade level they should be completed.
Universal Child Health Record - Form to be filled out by the parent of the child and their primary care physician certifying that the child is medically cleared to participate in school activities.It is always helpful to have an extra set of clothes available at school for any bathroom accidents.
- Please send a set in that is season appropriate including:
- shirt, pants/shorts, appropriate undergarments, and socks
- This set of clothing should be labeled with your child's name in a zip lock bag.
- All the above should be received on the first day of school

As a reminder, It is a law in NJ that all children ages four until age eight and less than 57" (4'9''), must be seated in a five-point harness car seat until they reach the upper limits of the seat, then in a belt-positioning booster seat in the rear seat of the vehicle.
If you need assistance obtaining a car seat, or have questions about car seat safety please contact your school nurse.
