• Important 5th Grade Information

    6th Grade Immunizations are required on the 1st day of school in September.

    As you enter Newcomb School for 6th Grade, there will be Immunization Requirements per the state of New Jersey

    • Students born on or after January 1, 1997 and entering Grade 6 will be required to receive a booster Tdap (tetanus, diphtheria, acellular pertussis) vaccine.  This requirement is designed to help reduce the number of outbreaks of pertussis (whooping cough) in previously vaccinated children in the 10-19 year age group.  This vaccine cannot be given earlier than the 11th birthday.
    • Students born on or after January 1, 1997 and entering Grade 6 will be required to receive one dose of a meningococcal vaccine to reduce the risk of bacterial meningitis. This vaccine cannot be given earlier than the 11th birthday.

    If you do not have health insurance or your health insurance does not cover immunizations, you may contact the Burlington County Health Department at 609-265-5526 to arrange for low-cost immunizations.  Southern Jersey Family Medical Center can also provide immunizations on a sliding scale fee and can be reached at 609-894-1100.

     

    Students with asthma, serious allergic reactions, seizures, or diabetes are required by state law to have action plans completed every school year. If these forms are not returned, your child will not be able to participate in any after-school activities (sports, clubs, and field trips).

    All physicals and medical forms must be turned into the nurse’s office. This cuts down on lost paperwork. We advise that you make copies for your records of any paperwork you send to the school
